The signal: what you noticed. If you came from a calculation, its result is already here.
One sentence. If you can’t, there is no decision yet.
On the review date: what happened and what you learned

    About the record

    What it is
One page per decision. The fields follow the chain: signal, decision, commitment, action, outcome, learning.
    Why these fields
A decision without an owner and a review date is not finished yet. That is why the card marks those items separately.
    What it does not do
The record does not judge whether the decision is good. It makes it checkable: on the review date you see what you expected and what happened.
    What next
On the review date, open the record from “My decisions” and fill in the last two fields.

When one record is not enough

If there are many decisions and they get lost between site meetings, what you need is not a record but a team habit: a decision log, ten minutes a week on open items and someone who looks after it.
